1. By signing this form, I release the St. Mary's University Mail Office from any and all liability for any loss or damage to mail in transit.
2. I understand that a time limit of six (6) months is imposed on this form, during which time it is my responsibility to notify any and all correspondents of my new address.
3. I understand that only first-class mail will be forwarded. Packages & foreign mail will be returned to sender.
4. I understand that any form received unsigned will result in return of any mail to sender.
5. If no Mail Forwarding Request Form is received from a resident, any and all first-class mail received for that resident will be returned to sender.
