Information Change
Students should verify the accuracy of their local/permanent address, telephone number, etc. on record with the Registrar's Office at the time of registration and report any changes thereafter. The Registrar's Office cannot accept an information change over the telephone or via e-mail. All information changes must be made in writing either in person or by mail or fax with the student's signature authorizing the changes. If students wish to mail or fax their information change request, we encourage them to use an information change form that can be downloaded from the Registrar website. If students do not have access to the web, the Registrar's Office can also accept an information change in the form of a letter. The letter should include the following:
- Full name
- Social Security Number and/or St. Mary's Identification Number
- Permanent Address and telephone number(Address at which the student resides while the University IS NOT in session)
- Local address (if different from the permanent address) and telephone number (Address at which the student resides while the University IS in session)
- Place of employment and work telephone number (optional)
- Cell telephone number (optional)
- E-mail (optional)
- Proposed graduation date (optional)
- Signature and the date
Name Change
In addition to updating their address and telephone number, students can also request a name change. Along with the information change form or letter requesting the information change, students must also submit a copy of their marriage certificate, divorce decree or court order to the Registrar's Office.
Upon requesting a name change, currently enrolled students should visit the Dean of Students Office to have a new St. Mary's identification card made. The Dean of Students is located on the second floor of the University Center. Please contact the Dean of Students at (210) 436-3714 for hours of operation.
Social Security Number Change
Any time a student wishes to change his/her social security number, a copy of the Social Security card must be submitted to the Registrar's Office along with an information change form or a letter requesting the information change.
Major Change
Students who wish to change or add a major should obtain a major change form from the Registrar's Office or the Registrar website. The form requires the signatures of the chairperson for the new major and the adviser for the new major. The chairperson will determine the student's new adviser. (A current departmental chairpersons list can be obtained from the Registrar website.) Upon obtaining these signatures, please return the completed major change form to the Registrar's Office in St. Louis Hall Room 104. Please check your Gateway account for the change to be made. Allow 1-2 days for processing.
Students leaving a major in engineering must visit with Graciela Lopez, Director of Advising in St. Louis Hall 105 before the Registrar's Office can accept the major change form. Ms. Lopez has an exit survey she must conduct for the purpose of evaluating the engineering program.
