Form Tips

• Adobe Acrobat Reader is required to open a PDF document. If you don't have Adobe Acrobat Reader, you can get Acrobat free of charge.
• You may use the "tab" key to move from field to the next as you complete the PDF document.
• To fill in a check box, click the right mouse button on the appropriate box.
• The "enter" key will allow you to enter a hard return on multi-line fields.
• Multi-line fields have limited space. If you need more space, attach a word processing document to the completed PDF document.
• Finally, print, sign and send via e-mail or fax. IMPORTANT: Call after faxing or ask for an e-mail response after e-mailing with attachment.
• If you are unable to key in the information in the PDF document, you may also print the form and hand write the information.
