Pre-matriculation Immunization Requirements

All students living on campus and all international students must submit a Health Record Form prior to the first day of class. Required immunizations, as outlined in the Health Record Form, should be completed prior to the first day of class. Students who fail to comply with this requirement will be reported to the Dean of Students and appropriate action will be taken. The immunization component of the Health Record Form must be signed by a physician or a copy of the original immunization record must be submitted. Student Health Center staff will be available at Orientation check-in to answer questions and collect forms. In addition, the Student Health Center will conduct an immunization drive for the following vaccinations:

Menactra (Meningitis) $125
Required for all freshmen and transfer students living on campus.

TB Skin Test (PPD) $15
Required of all international students and students outside of the United States.

Adacel (Tetanus, Diphtheria, and acellular Pertussis) $55
Required of all resident and international students.

Measles, Mumps and Rubella (MMR) $65
Required of all resident and international students.

Immunization fees will be charged to the student's account.
« See Forms for Download on the left side of the screen.

Please note that the CDC, ACIP and ACHA recommend that all college students, especially freshman living in dormitories become immunized against meningococcal meningitis to reduce their risk of getting this potentially fatal disease. Your signature is required on the Immunization Record confirming receipt of the meningitis information as mandated by the 77th Texas Legislature (2001) so don't forget to provide us with this.
