Supplemental Forms.

All the necessary Supplemental Forms (transcript request forms, recommendation forms, etc.) are included (HERE) in the Online Application. Every school has different Supplemental Forms, so you need to check your school's requirements within the application itself. Usually you need to send your Supplemental Forms to someone else (previous teachers, schools previously attended, etc.), then they send the Supplemental Forms to the school you are applying to. Print your Supplemental Forms and mail them where they need to go. Complete your application online and submit it, then send your Supplemental Forms in the mail.
