FERPA Third Party Authorization Form
According to federal law, a student's authorization is required for any account information to be release to a third-party.
This authorization form can be submitted at anytime and authorization is valid until cancelled in writing.

This deposit authorization is to remain in full force and effect until St. Mary's University has received written notification of its termination from the student and in such time and in such manner as to afford St. Mary's University and the depositroy a reasonable opportunity to act on it.

Meal Plans
Resident students can request a meal plan change by submitting a Meal Plan Change Form to Residence Life at ResLife@stmarytx.edu. no later than 5 pm on the first day of classes each semester. Commuter students can request to add a meal plan by completing the Request to add a meal plan form and submitting it to BusinessOffice@stmarytx.edu.
Outside Employer Payment Forms
Students taking advantage of any tuition reimbursement program must complete the Outside Employer Payment Acknowledgement form and send it to the Business Office by the appropriate payment deadline each semester.
Payments must be received no later than 30 days after the last day of class each semester. If payment is not received, a $150 late payment fee will be charged to the student's account.

Notice to Bill Texas Tomorrow Fund
Rates for Texas Tomorrow Fund for the 2012-2013 Academic Year
The Texas Guaranteed Tuition Plan, Texas' 529 Prepaid Higher Education Tuition Program, has established its hourly average tuition and required fee rates for the 2012-2013 academic year. They are as follows:
Private College Plan semester hour rate: $693.51
Senior College Plan semester hour rate: $265.96
Junior College Plan semester hour rate: $85.65
Based on authorized rates as stated on August 1, 2012

2012-2013 Academic Year
Request for payment are submitted to Texas Tomorrow Fund after the add and drop period. Payments are received in 30-45 business days.
The amount billed is based on the number of credit hours the students is enrolled for the semester. For example, if a student that has a Senior College Plan with Texas Tomorrow Fund is enrolled for 15 hours for the semester, the Plan will pay $3,989.40 ($265.96x 15 hours).
The amount paid is also limited by the hours remaining on an eligible beneficiary prepaid tuition contract with Texas Tomorrow Fund.
